By signing up below, you authorize the Black Hawk County Election Office to mail you an absentee ballot request form for the City/School Election to be held on November 4, 2025. Complete and return the form by the deadline listed below to receive your absentee ballot.

Request Form Deadline: Absentee ballot requests must be received by the office by 5:00 p.m. on Monday, October 20.
Ballot Mailing: Ballots will be mailed from October 15-20 for those who have submitted completed forms.
